National Referee Information

Referees who are registered with the United States Soccer Federation (USSF) must renew their registration annually. To re-certify or maintain your current referee grade follow the directions below and mail in all requirements with your registration form to the address at the end of this page. For referee renewal please read the 2010 Maintenance-Renewal requirements and for upgrades please read the 2010 upgrade requirements information below. Referees going Emeritus must send a note stating this with their registration form. Referees are registered for the calendar year, January 1 to December 31. Registration for the 2009 calender year is closed. Registrations must be submitted between September 1, 2009 to June 15, 2010, for the 2010 calendar year.

PLEASE ALLOW 4 WEEKS FOR PROCESSING AND APPROVAL. ONCE APPROVED YOUR NAME WILL APPEAR ON THE REGISTERED OFFICIALS LIST. USSF MAILS OUT THE PACKETS, THIS CAN TAKE UP TO 1 MONTHS FROM WHEN YOUR NAME IS LISTED ON THE WEBSITE.

RISK MANAGEMENT REQUIREMENTS
Please note all adult referees age 18 and up must have their Risk Management complete by January 1, 2010 to referee any youth games with Cal South. REFEREES MUST SUBMIT THE FOLLOWING:

·(Please note USSF will not be sending out the packet with registration forms anymore and you will or should have received a postcard reminder in the mail for 2010 registration)

1. Completed USSF Referee Registration Form

2. Along with the 2010 Maintenance-Renewal Requirements

3. Payment by check or Money Order, made payable to Cal South 2010 Referee Renewal Fees

Print 2 copies of the form, keep one for yourself and mail the other copy to the State.

Complete the form using one letter per square.

Complete as much of Step 1 as you can. If you are re-certifying, your USSF ID Number is required. If you don't know your USSF ID Number or can't locate it, leave it blank.

Current State Affiliation = CAS

Grade = Referee Grade, not your grade in school.

The DISTRICT field leave blank.

Check mark in the box in Step 2 if you are requesting an upgrade AND you meet all the requirements. 2010 Upgrade Requirements

Re-certifying Referees should report the number of matches done in the past year in Step 3, Affiliated Game Experience For Prior Year. Write in the actual number of games done at each level. Do not use "X's" or check marks.

Read Step 5 and then check the appropriate box. If you have checked the "yes" box, please submit the following information with your form: description of offense, date, court, and sentence. Information is evaluated on a case by case basis in relationship to being a referee. If a box is not checked in Step 5, the form will be returned.

Sign and date your form on the signature line provided. Your signature attests that all information contained on the form is true and correct. Misrepresentation or falsification of facts are reason for possible dismissal. Unsigned forms will be returned.

Check payable to "Cal South" in the appropriate amount.

Photocopies of physical fitness and written test results, if required.

Photocopies of certificates from clinics attended, if required.

Photocopies of Feedback to Official Assessment Report, if required.

List of games officiated during the previous registration year, if required.

Incomplete packages cannot be processed and will delay your registration.
Failure to include all required items in order to maintain your referee grade may result in the downgrading of your referee license.
