Reminders

1. Each Manager must check-in their team with National Cup Officials1 1/2 hours prior to the teams first game.
   
2. Print and read Cal South National Cup Tournament Rules and Regulation document
   
3. Original Birth Certificates are required beginning with elimination round of play. No copies will be accepted, no exceptions!! Pasport is also an acceptable form. For olders, Drivers License can be used as well.
   
4. All teams with Foreign Language Birth Documents must have the document translated and use Foreign Document Translation Formprovided by Cal South. The form can be found on the Cal South web site under resources/forms or by clicking on the above underlined text
   
5. Print a copy of the Schedule for your respective Team from the National Cup Schedules site
   
6. At check-in the Team Managers will be instructed on the procedure for the Roster Game Form
   
7. Please check the Red Cards report on the "Schedule & Results" page to determine if your team has players and or coaches who are ineligible to play or coach
   

Required at Registration

The person checking in the team should be prepared with the following information required to complete the Game Roster Forms and check-in the Team. Bring a pen, you will need it!!

• Pen
• Game Dates (these can be obtained from the schedules)
• Name of the playing venue
• Field #
• Start Time
• Opponent
• Jersey Color (Home and Away for your team)
• Jersey Numbers (Home and Away)
• Team Manager Name
• Team Manager Phone Number (possibly more then one)
• Coach Name
• Coach's License Level (#)
• Coach Phone #
• Home or Away designation (Team listed first on the schedule is the Home team)
• Birth Certificates or Passports - not for bracket play, but prior to the first elimination round game
• Original Medical Release Forms for each player - must have orignal (wet) signatures
• White Player Cards (Temporary cards will NOT be accepted)
